As the home health care industry has grown and expanded, the need for a planned, organized approach to infection control has become increasingly important.  Home health aides provide a wide range of care to home bound patients and must be aware of the routes of disease transmission, the importance of using Standard Precautions to decrease job related exposures to infectious diseases, and proper hand hygiene.  This program is designed to provide the basic principles necessary for infection control to be successful and how these principles are applied in the health care setting.

As a result of the HIV epidemic in the mid-1980’s the concept of Universal Precautions was developed by the Centers for Disease Control and Prevention. In 1991, the Occupational Safety and Health Administration (OSHA) published the Final Rule to improve working conditions and to promote safety for healthcare workers when caring for patients infected with blood-borne pathogens.  The focus of this program is to review the current guidelines for infection control practices; learn how to identify strategies for reducing antibiotic resistance within the population; and present current practices for controlling MRSA.

The older you get, the more important it is to take care of your skin as the skin becomes thinner, begins to sag, causes wrinkles and injures more easily and heals more slowly.  Proper skin care is important for both our physical and mental health.  For instance, skin infection in a diabetic patient can lead to the serious complication of gangrene.  Those who are bedridden need to avoid prolonged pressure on the ankles, heels and buttock.  Too much pressure can tear the thin skin leading to bedsores.  And itchiness, a very common problem among the elderly, can be so irritating as to affect how we feel.  Therefore, the elderly and caregivers should know about proper skin care.

Botulinum toxin has been evaluated by offensive biological weapons programs in the past, including the former U.S. program, the former Soviet Union program, and the former Iraqi program.  This two-hour satellite broadcast will inform and educate health professionals with detailed information on botulinum toxin.  World-renowned experts from the U.S. Army Medical Research Institute of Infectious Diseases (USAMRIID) and medical toxicologists with clinical experience treating cases of botulism from a variety of other organizations will present this program.

Deep Vein Thrombosis (DVT) is a common but serious medical condition that occurs when a thrombus (blood clot) forms in one of the large veins, usually in the lower limbs, leading to either partially or completely blocked circulation.  According to the American Heart Association, DVT occurs in approximately two million people and up to 600,000 are hospitalized in the US each year.

Scientists predict that the world is due for an influenza pandemic—a global outbreak from a new strain of flu—that would have a dramatic impact on our nation’s communities, schools, and families.  History tells us that flu pandemics are a reality: in the past century alone, three of them caused widespread loss of life and the wholesale disruption of societies small and large.  A flu outbreak today similar to the one that occurred in 1918 could potentially kill millions and profoundly affect institutions such as our nation’s school systems, which are teeming with children who are expected to have high rates of infection and are more likely than adults to spread the illness.

An influenza pandemic occurs when a new virus emerges for which people have little or no immunity, and for which there is no vaccine.  The disease spreads easily from person-to-person, causes serious illness, and can sweep across the country and around the world in a very short time.  While it is difficult to predict when the next influenza pandemic will occur, during the past 100 years, there have been three major pandemics: 1918-1919, 1957-1958, and 1968-1969.

Kidney disease is the ninth leading cause of death in the United States. According to the National Institute of Health’s National Kidney Disease Education Program there are more than 20 million Americans with kidney disease. In 1970, Title X of the Public Health Service Act was enacted by Congress with bipartisan support and signed into law by President Nixon.  Title X was designed to make contraceptive supplies and services available to all who wanted and needed them but who were unable to afford them without government assistance.  The intent of this legislation was to make readily available a broad range of effective and acceptable family planning methods on a voluntary and confidential basis.

The role of lipids in the development of atherosclerosis has clearly been demonstrated.  Accordingly, CVD primary prevention strategies need to be started at an earlier age. Implementing strategies to identify children at high risk for CVD and effectively minimize risk factors is of the utmost importance if we are to curtail the early onset of atherosclerosis and slow progression to CVD. The American Heart Association (AHA) has provided guidance regarding the identification of children at risk for CVD and is in the process of developing a scientific statement to address the medical management of hyperlipidemia in high risk adolescents.

Many people are perhaps unaware that they are living near a meth lab.  Most labs are found in rural cities and suburban residences.  Individuals involved in these illegal activities often use barns, garages, and other out buildings, back rooms of businesses, apartments, hotel and motel rooms, storage facilities, vacant buildings and sometimes vehicles.  These labs are characteristically small and make use of common household appliances, glassware and chemicals that are readily available.  The focus of the presentation is to make the Home Health Aides and Home Attendants aware of the dangers and signs and symptoms of a meth lab in or around their patients’ homes.

Federal Department of Transportation (DOT) and International Air Transport Association (IATA) regulations require that any person who packages and ships these types of specimens must be trained in awareness of the regulations.  Many changes to these regulations occurred in 2005 and this program will outline these changes and cover how they affect county personnel who package and ship specimens.  If packaging and shipping falls within an employee’s duties, the employee must have documentation of awareness training for these regulations.

The impact of pandemic influenza will be felt in every community by every citizen.  Hospitals will be overwhelmed and pharmaceutical interventions will be in short supply.  Critical infrastructure such as transportation, commerce, utilities, and public safety will be disrupted.  With simultaneous outbreaks occurring over large geographic regions, communities will have to rely on local resources and coordinate their preparedness activities across all sectors.  This program will examine the case of California’s Santa Clara County, where the community is preparing for a pandemic that will challenge their public health system and affect every aspect of their daily life.

Of the estimated 1.2 million coronary attacks to be experienced by Americans in 2006, almost half (500,000) will be recurrent events.1  Accordingly, secondary prevention is an essential part of the comprehensive care of the patient with coronary heart disease (CHD).  The American Heart Association (AHA) and the American College of Cardiology (ACC) recently issued new, evidence-based guidelines regarding the management of atherosclerotic disease, an important CHD risk factor, in patients following a heart attack.

Title X providers are legally obligated to provide confidential healthcare services to adolescents.  At the same time, they are also required to comply with state laws for the reporting of statutory rape, abuse and neglect of minors.  The Title X Grant requirements state that the grantee must provide counseling to minors on how to resist attempts at being coerced into engaging in sexual activity.  This program will define sexual coercion, outline specific counseling characteristics of various ages of teens, cite current Alabama law and reporting guidelines, and show you how to develop a work plan for integrating sexual coercion counseling in a clinic setting.

Stable angina, chest pain caused by insufficient blood flow to the heart muscle, is a common disorder affecting approximately 6.5 million people in the United States.  Although associated with diminished quality of life and increased risk for future cardiovascular events, stable angina is often not perceived as an eminent threat.  Practice guidelines for the management of chronic stable angina were published by the American Heart Association (AHA) and the American Stroke Association (ASA) in 2003. However, guideline-recommended treatments for stable angina are underutilized and anginal symptoms are not adequately controlled in nearly a third of patients.

“What School Counselors Need to Know” - This presentation will provide a brief overview of suicide in America, the goals and objectives of the Surgeon General’s National Strategy for Suicide Prevention, 2001, and how developing social policy is impacting the training and practice of school counselors.  Also included is a review of the relationship of untreated mental illness and suicide risk in youth and a description of the QPR Suicide Risk Reduction Program for Schools.
“What Mental Health Practitioners Need to Know” - 
This presentation will provide an overview of suicide prevention and risk assessment, including the latest research on scope of problem and recommended strategies for prevention and risk reduction; describe the risk relationship of mental illness and substance abuse to suicide attempts and completions; identify implications for management and practice using a framework of practical strategies including risk identification and referral, “gatekeeper” concept; describe risk assessment strategies and a systemic approach to risk reduction including frameworks and tools.

“Suicide Bombers” Exactly when a suicide bombing will take place on American soil is unknown.  Intelligence and surveillance are vital for police to safely apprehend suicide bombers and prevent detonation.  No suicide bomber profile exists; however, there is a behavioral profile that may be used to assist police and other emergency responders in recognizing them.  We will look at the advantages of suicide bombers, pre-incident indicators, and suicide bombing incident response.
“Medical Surge” If a bioterrorism incident occurs, many people may need treatment in their communities.  Providing the help that they need quickly and efficiently will save lives.  We will explore potential resources that may be needed in a medical surge incident, including mass prophylaxis
“Critical Resource Logistics & Distribution”
In the event of a natural or manmade disaster, critical resources will be necessary for preservation of life, safety, and security.  Preparedness plans must include a way to identify, dispatch, mobilize, accurately track, and demobilize available human and material resources across all incident phases.

Stable angina, chest pain caused by insufficient blood flow to the heart muscle, is a common disorder affecting approximately 6.5 million people in the United States.  Although associated with diminished quality of life and increased risk for future cardiovascular events, stable angina is often not perceived as an eminent threat.  Practice guidelines for the management of chronic stable angina were published by the American Heart Association (AHA) and the American Stroke Association (ASA) in 2003. However, guideline-recommended treatments for stable angina are underutilized and anginal symptoms are not adequately controlled in nearly a third of patients.

“Teen Depression” -  David Shaffer, MD, a world-renowned child psychiatrist and expert on adolescent suicide, will present a comprehensive overview of what we know about the underlying causes of youth suicide and the early identification on an evidence-based mental health and suicide risk screening program, developed as a direct outgrowth of his research on adolescent suicide.
“Self Injurious Behavior”
-  It is widely believed that the number of youth who intentionally injure their bodies by cutting, burning, or engaging in other forms of mutilation has increased in recent years.  Join Dr. Janis Whitlock as she summarizes what we know and don't know about self-injury.  In addition to sharing what is known about the prevalence, nature, and contagiosity of self-injurious behavior in the general youth population, she will share results from a New York State study of self-injury in schools.  The talk will end with recommendations for detection, intervention, and prevention of self-injury in adolescents and young adults.

There now is convincing evidence to demonstrate that high cholesterol and low high-density lipoprotein levels during adolescence initiate and accelerate the pathologic changes in coronary arteries that lead to atherosclerosis.  Implementing strategies to identify children at high risk for CVD and effectively minimize risk factors is of the utmost importance if we are to curtail the early onset of atherosclerosis and slow progression to CVD.  The American Heart Association (AHA) has provided guidance regarding the identification of children at risk for CVD and is in the process of developing a scientific statement to address the medical management of hyperlipidemia in high risk adolescents.

A sexual history is important for all patients because it provides information identifying high-risk behaviors  leading to an unintended pregnancy and/or a sexually transmitted infection, including HIV.  Taking a thorough sexual history can also be a gateway for direct essential risk reduction counseling.  Many patients, and sometimes the clinicians, are embarrassed or feel uncomfortable talking about sexual issues.  It is important for the provider to be competent and professionally skilled in talking with the patient(s) in a non-threatening manner, to find out what problems exist, and uncover the appropriate background information and clinical history.  With the right consultation tools, a sexual history can be performed efficiently when time is a critical factor in the clinic setting.

State and local emergency planners are in the process of designing their Points of Dispensing, or PODs.  Planners have been calculating the number of PODs required too provide prophylaxis to the entire population within 48 hours and the number of people per hour (PPH) that must be served in each POD in order to achieve this 48-hour goal.  This broadcast will explore ways in which computer simulation and modeling software can assist planners in designing the most efficient PODs.  Panelists will discuss several available computer models, how modeling fits in to the overall exercise program, how modeling can be used during an incident, and ways in which modeling can take some of the guesswork out of POD design and staff allocation.

Drugs cause people to do strange things.  While some drugs make people feel mellow, others lead to hyperactivity and everything in between. Stable angina, chest pain caused by insufficient blood flow to the heart muscle, is a common disorder affecting approximately 6.5 million people in the United States.  Although associated with diminished quality of life and increased risk for future cardiovascular events, stable angina is often not perceived as an eminent threat.  Practice guidelines for the management of chronic stable angina were published by the American Heart Association (AHA) and the American Stroke Association (ASA) in 2003. However, guideline-recommended treatments for stable angina are underutilized and anginal symptoms are not adequately controlled in nearly a third of patients.

Hurricanes are just one example of a catastrophic event when the public health workforce must be ready to set up, staff and manage shelters for those displaced by the disaster.  Maintaining appropriate environmental health standards is one of many considerations for those responsible in setting up the shelters.  Broadcast faculty will examine the example of two mega shelters set up in Houston in the aftermath of Hurricanes Katrina and Rita.  Issues to be discussed include food safety, housekeeping and sanitation procedures, infection and outbreak control, pet shelters, and communication and staffing, including the screening and management of volunteers.
