According to a study published by the National Center on Addiction and Substance Abuse at Columbia University, less than 20% of physicians consider themselves “very prepared” to diagnose alcohol dependence.  In sharp contrast, a large proportion of these same physicians feel “very prepared” to identify hypertension (83%), diabetes (82%), and depression (44.1%).  When asked about treatment, researchers found that most physicians feel treatment is “very effective” for hypertension (85.7%), diabetes (69%), and depression (42.5%), but only a few feel treatment is “very effective” for alcoholism (3.6%).(2)

This presentation looks at the impact of various public health disasters on both the system and the emotional dynamics of caretakers. We distinguish between disasters, where resources were stressed but supported (Tokyo sarin attack) vs. those where systems were stressed and unsupported (Ebola outbreak in Kinshasa, Democratic Republic of the Congo), 1918 worldwide pan flu epidemic, a worldwide SARS epidemic, possible nuclear contamination, and natural disasters, such as Hurricane Katrina.

The Alabama Department of Public Health is committed to providing our employees and clients a safe workplace free from harassment, intimidation, threats, and violence.  Employees are required to read and follow the “Policy Against Violence in the Workplace.” All supervisors must attend the “Interview and Selection” training to ensure prescreening steps are followed in the hiring process.  An employee who is going through a difficult time should be reminded and encouraged to participate in the State’s Employee Assistance Program (EAP). Supervisors must address unprofessional conduct in the workplace before it escalates to more serious problems.

In addressing the emerging and re-emerging challenges of public health in the 21st century, CDC is focusing efforts to improve the health of Americans through a more integrated and comprehensive approach.  CDC’s Healthy Places goals examine a broad spectrum of health issues associated with communities, homes, schools, workplaces, healthcare facilities, institutions, and transportation/recreational facilities. This approach seeks to address the potential human health impacts of physical space and environmental exposure.  Healthy communities are places where public health systems, social infrastructure and policies support health and essential public health services (including quality health care) that are readily available to all.

Public health leaders are already involved in a wide array of health promotion, prevention and disease management programs from AIDS to zoonoses, anti-smoking and immunization campaigns, safe food and water inspections, plus bioterrorism preparedness.  But now, as we learn more about the social determinants of health: education, income, housing and transportation – we need to examine an even broader framework for public health action in our communities, large and small.  This program will document and celebrate the important work and achievements of public health today. It will also examine future needs and challenges.

Stroke  is a leading cause of serious, long-term disability, is the third leading cause of death, and is associated with an estimated annual cost of $62.7 billion.  Prevention remains the best approach for reducing the overall burden of stroke.  Implementation of effective prevention strategies and the corresponding reduction in risk factors can have a significant impact on reducing the incidence of stroke.  Primary prevention is particularly important because greater than 70% of strokes are first events.

Many studies show that infants born at a healthy interval are more likely to survive, less likely to be premature or low birth weight, and less likely to be malnourished.  Longer birth intervals are not only healthier for mothers and their children but it enables parents to devote more time to each child in the early years, gives parents more time for activities other than child rearing and often eases pressure on family finances.  Couples and individuals need to make their decisions on birth spacing based on accurate information and with a range of contraceptive options.  Healthcare providers and family planning programs have a responsibility to help them.  Although not always addressed specifically, promoting birth spacing has long been a central goal of family planning programs.

“Historical Roots of Bio-Terrorism” - 1000-1200 ET - A historical review of bioterrorism begins with the Sumerian tablets around 3000 BC that relate knowledge of contaminated objects leading to pestilence.  The September 11th attacks and the subsequent anthrax letters have focused renewed attention on modern day terrorism involving chemical, biological, radiological, or nuclear weapons.          "
Bio-Security: Perception is Not Reality" - 1300-1430 ET -
The incidences of infectious disease events are increasing worldwide at a time when there are more restrictions placed on the use of veterinary drugs and a growing concern by the public over the safety of the food supply.  To address this situation, the emphasis has turned towards enhanced biosecurity.  We have known for years how to biosecure premises but implementation of these measures is inconsistent.  This is true at different levels in production systems.

“Under the Influence”; 1000-1100 - While some drugs make people feel mellow, others lead to hyperactivity and everything in between.  Usage of multiple types of drugs mixes the symptoms, making things even more difficult.  What's a first responder, new law enforcement officer, parent, teacher, or concerned citizen to do when encountering someone who is exhibiting strange behaviors that you just can't figure out?
“Interdiction Introduction”; 1100-1200

Can you imagine life without the internet and email?  While most people send pictures and emails and chat in online social networking groups to make friends, some non law-abiding citizens use these methods to plot their next illegal activity.  The fact that many gang members, drug dealers, and other criminals post to online networking groups is a great thing for law enforcement.  During this hour-long training broadcast, learn from law enforcement experts about all the information that is available on online social networking sites and how to get it.  They'll also talk about how to make your case with the information you find online, and how to turn it into face-to-face busts.

The role of lipids in the development of atherosclerosis has clearly been demonstrated.  Accordingly, CVD primary prevention strategies need to be started at an earlier age. Implementing strategies to identify children at high risk for CVD and effectively minimize risk factors is of the utmost importance if we are to curtail the early onset of atherosclerosis and slow progression to CVD. The American Heart Association (AHA) has provided guidance regarding the identification of children at risk for CVD and is in the process of developing a scientific statement to address the medical management of hyperlipidemia in high risk adolescents.

Scientists predict that the world is due for an influenza pandemic—a global outbreak from a new strain of flu—that would have a dramatic impact on our nation’s communities, schools, and families.  History tells us that flu pandemics are a reality: in the past century alone, three of them caused widespread loss of life and the wholesale disruption of societies small and large.  A flu outbreak today similar to the one that occurred in 1918 could potentially kill millions and profoundly affect institutions such as our nation’s school systems, which are teeming with children who are expected to have high rates of infection and are more likely than adults to spread the illness.

The impact of pandemic influenza will be felt in every community by every citizen.  Critical infrastructure such as transportation, commerce, utilities, and public safety will be disrupted.  With simultaneous outbreaks occurring over large geographic regions, communities will have to rely on local resources and coordinate their preparedness activities across all sectors.  Networks and relationships have to be established to communicate and manage the risks, now and during the pandemic.  This program will examine the case of California’s Santa Clara County, where the community is preparing for a pandemic that will challenge their public health system and affect every aspect of their daily life.

An influenza pandemic occurs when a new virus emerges for which people have little or no immunity, and for which there is no vaccine.  The disease spreads easily from person-to-person, causes serious illness, and can sweep across the country and around the world in a very short time.  While it is difficult to predict when the next influenza pandemic will occur, during the past 100 years, there have been three major pandemics: 1918-1919, 1957-1958, and 1968-1969.

Public health professionals responsible for pandemic flu planning are working at a distinct disadvantage.  While the public is the object of their concern, strategies and plans are being developed without directly involving the public.  Building on findings from the Redefining Readiness study and a new community engagement process in the four Redefining Readiness demonstration sites, Dr. Lasker will show how the public's knowledge and perspectives can help public health professionals deal with some of their most vexing challenges.

Program # 1 “Risk Factor Controls for Stroke” 1100-1200 - Recurrent stroke continues to be an important public health concern—almost a third of the estimated 700,000 strokes that occur each year in the United States are secondary strokes.  Substantial evidence indicates that recurrent ischemic stroke can be prevented.  An effective prevention strategy involves management of all modifiable risk factors and requires lifestyle, medical and, possibly, surgical intervention. Program #2 “Ischemic Stroke: Risk Factors & Prevention”  1200-1300 - Prevention remains the best approach for reducing the overall burden of stroke.  Implementation of effective prevention strategies and the corresponding reduction in risk factors can have a significant impact on reducing the incidence of stroke.  It is estimated that up to one half of all strokes could be prevented through stroke risk factor detection and management.

The food industry, including livestock, is one of the most open industries in North America, and is readily susceptible to outbreaks of Foreign Animal Disease (FAD).  FAD could be introduced accidentally - as happened in the UK - or it could be introduced intentionally.  History has illustrated that during an FAD, there is massive and widespread depopulation of millions of animals and a major disruption to most related livestock industries.  There also are significant ill effects on domestic and export markets for meat and milk, and the potential for a downward spiral for the national economy.

“Utilizing Government Resources in a Disaster”  - The expectation in emergencies is that all citizens affected in the disaster area receive prompt appropriate care.  Hurricane Katrina caused extensive and severe damage over the southeastern United States, including the entire Mississippi Gulf Coast on August 29, 2005.  Federal disaster declarations blanketed 90,000 square miles of the United States, including 49 counties in Mississippi. Hurricane Katrina has been classified as the worst natural disaster to affect the United States to date.
“Planning and Execution of Disaster Response”  -
E
ffective response to a terrorist WMD attack depends, in large part, on the capability of local responding agencies to rapidly focus available assets in accordance with valid operational plans to save lives and mitigate injury to the greatest extent possible.   This program proposes a framework for emergency management and response based on the principles of counter-terrorist preparation and incident command.  Following this framework will result in the timely marshalling and effective utilization of available assets in accordance with pre-established plans reflecting accepted guidelines and standards within a logistics, personnel and communications infrastructure that is designed to ensure robust and reliable intervention.

“Drug Epidemics: From Meth to Mothballs” ; 1000-1100 ET  -  Methamphetamine is getting lots of media attention recently, but is it really the "hot drug" in your community?  Young people and others are using many different kinds of drugs to get high, some you may not have even heard of.  How do you know what is really causing the problems in your community?  It might be the drug that's the most well-known, or it might not be.
“Cracking the Cocaine Habit”;
1100-1200 ET  -
Cocaine was the status drug of the 1980s and then crack became the drug epidemic of the 1990s.  Cocaine and crack are still out there, causing damage to the people already using and tempting those who aren't.  Powder cocaine is quickly becoming the glamour drug of choice again.  Crack cocaine is still ravaging communities, with no end to the cycle in sight.

Weight gain is an important contributing factor to metabolic syndrome, as it is a key component in the development of diabetes mellitus and cardiovascular disease.  In patients with psychiatric disorders, mortality linked to metabolic syndrome is compounded by behaviors such as heavy smoking, sedentary lifestyle, and poor eating habits.  Weight gain can affect medication adherence and data continues to emerge on the link between diabetes and cognitive dysfunction, all of which can negatively impact patient outcomes.  Treatment with antipsychotic agents may also play a role in weight gain.  This problem highlights several important needs: careful screening to recognize factors that may contribute to metabolic syndrome, appropriate selection of an antipsychotic agent, systematic monitoring, and effective management.

Many people are perhaps unaware that they are living near a meth lab.  Most labs are found in rural cities and suburban residences.  Individuals involved in these illegal activities often use barns, garages, and other out buildings, back rooms of businesses, apartments, hotel and motel rooms, storage facilities, vacant buildings and sometimes vehicles.  These labs are characteristically small and make use of common household appliances, glassware and chemicals that are readily available.  The focus of the presentation is to make the Home Health Aides and Home Attendants aware of the dangers and signs and symptoms of a meth lab in or around their patients’ homes.

Public health officials and members of the business community are discovering the mutual benefits of partnering to get Pills into People.  Bound together by a common goal—protecting the health and safety of the citizens of the community—public health and business are finding new strategies to solve some of the problems inherent in a mass antibiotic dispensing campaign.  Both sides of this multidisciplinary partnership have something unique to offer and also to gain in this relationship.  For example, when local businesses are willing to dispense antibiotics to employees and their families, the number of people who will report to community  Points of Dispensing (PODs) can be significantly reduced.  This relieves some of the burden on the PODs and offers businesses the opportunity to protect their employees and maintain continuity of operations.  This broadcast will focus on the development of these public-private partnerships.  Panelists will discuss methods for locating and reaching-out to businesses and identify significant challenges, possible solutions, and opportunities involved in this type of community outreach.  In addition, business leaders and public health professionals will share their experiences in forming these partnerships.

Program #2 “Ischemic Stroke: Risk Factors & Prevention”
  1200-1300 -
Prevention remains the best approach for reducing the overall burden of stroke.  Implementation of effective prevention strategies and the corresponding reduction in risk factors can have a significant impact on reducing the incidence of stroke.  It is estimated that up to one half of all strokes could be prevented through stroke risk factor detection and management.

A sexual history is important for all patients because it provides information identifying high-risk behaviors  leading to an unintended pregnancy and/or a sexually transmitted infection, including HIV.  Taking a thorough sexual history can also be a gateway for direct essential risk reduction counseling.  Many patients, and sometimes the clinicians, are embarrassed or feel uncomfortable talking about sexual issues.  It is important for the provider to be competent and professionally skilled in talking with the patient(s) in a non-threatening manner, to find out what problems exist, and uncover the appropriate background information and clinical history.  With the right consultation tools, a sexual history can be performed efficiently when time is a critical factor in the clinic setting.

In 1988, the Centers for Disease Control and Prevention initiated the Youth Risk Behavior Survey in order to process and identify the leading causes of mortality, morbidity, and social problems among our youth. These behaviors were identified and categorized into six risk areas: 1) behaviors that result in unintentional and intentional injuries; 2) tobacco use; 3) alcohol and drug abuse; 4) sexual behaviors that result in HIV infection, other sexually transmitted diseases, and unintended pregnancies; 5) physical inactivity; and 6) dietary behaviors. The collected data is assembled by surveying Alabama 15–19 year olds every other year. The trends that are recorded are compared to the National Average and studied by educators and health professionals to determine the prevalence of health-risk behaviors.
