a. Sponsors: This program is co-sponsored for
Military and Federal GETN subscribers by The Arthritis Program of the U.S.
Centers for Disease Control and Prevention and the Directors of Health Promotion
and Education and produced by the Alabama Department of Public Health and
Department of Defense Medical Interagency Satellite Program with the technical
support of the Satellite Education Network (SEN) at Ft. Lee, VA.
b. Availability: This program will be available to military and federal
GETN/MIST sites that have GETN/ATN/Warrior digital satellite downlinks. A
listing of potential GETN receive sites can be seen here:

MIST affiliate satellite networks such as the VA’s Employee Education System and the Department of Justice Television Network may also carry this program but IAW with their own scheduling.
There is C/Ku Band analog satellite availability for this program and coordinates will be included in the final confirmation message sent 24-48 hours prior to broadcast.
Videoconferencing may be available to selected military and federal VTC sites using the USA TNET or the USN CNET networks and other VTC networks able to connect to this satellite feed. Site coordinators using VTC are responsible for their own bridge coordination.
Will this program be Webcast? Yes, instructions in final confirmation sent out 24 – 48 hours prior.
c. Target Audience Statement: Program decision makers, their partners and potential partners, federal and military professionals and other professionals with an interest in elder health and community-based chronic disease programs.
d. Program Description: This broadcast is specifically designed to inform program decision makers about the supporting science, benefits and implementation essentials of the Chronic Disease Self-Management Program. CDSMP is a cost effective self-management education program that allows organizations to provide self-management support across multiple diseases and conditions. The broadcast will present the important issues for those considering adopting the program. Faculty will share their perspectives and pearls regarding the models their communities use to successfully disseminate and sustain CDSMP.
e. Program Objectives:
1. Provide a brief description of the Chronic Disease Self-Management Program.
2. Describe the evidence-base supporting the use of the CDSMP.
3. Discuss strategies for engaging partners (who, how to attract interest, making it a win-win).
4. Describe models for CDSMP program management/ oversight and support.
5. List infrastructure needs to support and sustain CDSMP.
